Bernal Heights takes its name from José Cornelio Bernal (1796–1842), whose family descended from one of the soldiers in Juan Bautista de Anza's 1776 colonizing expedition that established the Presidio of San Francisco and Mission Dolores. In 1839, the governor of Alta California awarded Bernal the Rancho Rincon de las Salinas y Potrero Viejo — a 4,446-acre Mexican land grant encompassing roughly 20 percent of what is now the city of San Francisco, stretching south from present-day Cesar Chavez Street to Daly City. Bernal built his family homestead near the site of what is now St. Luke's Hospital.

The neighborhood that bears his name grew gradually through the second half of the 19th century, settled first by Irish, Swedish, and German immigrants who farmed the land and ran dairy ranches on the open hilltop pasture. The completion of the San Francisco to San Jose rail line in 1863 linked Bernal Heights to the broader city, and the 1906 earthquake proved a turning point — built atop seismic-dampening radiolarian chert bedrock, most of Bernal's structures survived the tremor and the fires that followed, drawing displaced residents from across the city to the hill's stable ground. The Alemany Farmers' Market, operating every Saturday at the neighborhood's southeast corner since August 1947, remains one of the oldest continuously operating open-air markets in the United States.
