Forest Glen takes its name from the wooded community that grew up around Captain William Cross Hazelton's farm, a Civil War veteran who settled here in 1866 and built the first house, barn, and general store in the area over the following decades. Hazelton, a strict Congregationalist, personally screened every family before allowing them to settle nearby, a practice that slowed the community's growth but shaped its character for generations. Forest Glen was one of the last areas annexed to Chicago, joining the city as part of Jefferson Township in 1889.

Hazelton's own house, moved to its current location in 1920, was designated a Chicago Landmark in 2005 and still faces the Forest Glen Forest Preserve, land Hazelton himself donated to the community. The First Congregational Church he helped found continues to serve the neighborhood, and the Forest Glen Metra station still carries commuters from the same rail corridor that first drew settlers to this stretch of prairie.
