Forest Knolls takes its name from the forested knolls of Mount Sutro's south slope, where the neighborhood was carved out of the eucalyptus forest Adolph Sutro planted in the 1880s. As late as 1905, the area was described in a San Francisco Call article as rural terrain accessed by the "Almshouse Road" — the unpaved path now known as Clarendon Avenue — running past reservoirs and wooded slopes to the Almshouse at what is now Laguna Honda Hospital. The neighborhood was largely developed for housing in the late 1950s and early 1960s, when developer Carl Gellert graded and scraped the hillside bare before constructing the compact streets and hillside homes visible in a 1961 photograph taken at the dedication of the adjacent Midtown Terrace Playground. Forest Knolls was among the last West of Twin Peaks neighborhoods to be developed, marketed to middle-class buyers at a time when racial diversity was finally arriving in the western neighborhoods after decades of restrictive housing covenants.

The neighborhood sits on the southwestern slopes of Mount Sutro at 918 feet — one of the highest residential elevations in San Francisco — with UCSF's main campus occupying the forested summit above it. Its hillside stairways, which link the steep streets to Clarendon Avenue below, give Forest Knolls a character that neighbors have described as "Marin-like" for its density of trees and sense of remove from the urban grid.
