Laguna Honda takes its name from a Spanish phrase meaning 'deep lagoon,' referring to a natural spring-fed pool on the slopes behind Twin Peaks that once supplied fresh water to the surrounding land. The neighborhood's identity is inseparable from Laguna Honda Hospital, which was founded in 1866 as an almshouse on the old Rancho San Miguel — formerly the property of José de Noe, the last alcalde of San Francisco during the Mexican era. The institution, built to care for Gold Rush pioneers who had come west seeking fortune and gone bust instead, was the city's response to a growing population of the sick, elderly, and destitute. It was also known as the Relief Home after the 1906 earthquake, when thousands of displaced San Franciscans were housed and cared for on its 62-acre campus. President Theodore Roosevelt dedicated a new 1,000-bed pavilion here in 1909, and the Spanish Revival buildings dedicated in 1926 remain prominent landmarks on the hillside above Woodside Avenue.

The neighborhood is also known as Forest Hill Extension, reflecting its proximity to one of San Francisco's historic residence parks. The Laguna Honda Reservoir, a municipal water infrastructure landmark, and the Muni Metro Forest Hill Station at the neighborhood's edge connect Laguna Honda to the broader city. The hospital itself became accredited in 1963 and today serves as the city's long-term acute care facility on an 87-acre campus.
