Lower Pacific Heights, also historically called Upper Fillmore, occupies the south-facing slope below Pacific Heights proper, bounded by California Street to the north, Geary Boulevard to the south, Presidio Avenue to the west, and Van Ness Avenue to the east. Historically part of the Western Addition, the neighborhood developed as a middle-class residential district in the late nineteenth and early twentieth centuries — geographically and socially intermediate between the mansions of Pacific Heights and the working-class Fillmore. The area was built up primarily with Victorian and Edwardian flats and apartment buildings in the decades following the 1906 earthquake, when the Western Addition's survival of the fire made it a destination for displaced residents. Fillmore Street, named for President Millard Fillmore and platted as part of the Western Addition in the 1860s, forms the neighborhood's commercial spine.

The neighborhood's name shift from Upper Fillmore to Lower Pacific Heights came in the early 1990s as San Francisco real estate values escalated, with agents seeking to capitalize on Pacific Heights' prestige. The Madame C.J. Walker Home for Girls and Women at 1320 Scott Street, a San Francisco Designated Landmark, housed a charitable organization supporting single African American women new to the city from 1921 to 1972. The Japan Center complex and Peace Pagoda mark the neighborhood's southeastern boundary.
