Pill Hill takes its name from the doctors — many from nearby South Chicago Community Hospital — who began settling atop the neighborhood's namesake ridge in the 1960s, when "pill" was slang for medicine and its practitioners. The land had been swampy farmland through the nineteenth century before factory workers developed it in the 1870s; Chicago annexed the area along with the rest of Hyde Park Township in 1889. By the 1940s, white-collar professionals had replaced the original factory workers, setting the stage for the affluent enclave that emerged three decades later.

The neighborhood sits atop Stony Island Ridge, a limestone formation that was once an island when glacial Lake Chicago covered the region thousands of years ago. Within its compact eight-block footprint — bounded by 91st and 95th Streets, Cregier and Paxton Avenues — Pill Hill preserves one of Chicago's best-kept collections of 1950s and 1960s "atomic-era" homes, each individually designed with angled roofs, stone facades, and broad picture windows, a deliberate contrast to the uniform bungalows found elsewhere on the South Side.
